Below is a list of items that are required for registering with the City of Mulberry.
- A copy of a current Business Tax Receipt from a county of city in the state of Florida.
2. General Liability Insurance Certificate. Worker Comp insurance certificate or exemption in the license holders name.

3. Accela Associated User Form optional for the license holder to give others access to apply for permits with their license.
4. Complete the State Certified Registration Form

Private Providers
If you are using a Private Provider for your project, all forms below shall be submitted at the time the permit is
applied for.
All forms will be provided by the City of Mulberry. Unless otherwise provided for by an applicable Florida law, no substitutions will be accepted.
Section 553.791(16) of the Florida Statutes, any notice to be provide by the City of Mulberry Building Official shall be deemed to be provided to the
person or entity when successfully transmitted to the email address listed for that person or entity in the permit application or revised permit application.
In the event an email address is not provided or listed on a permit application, it is acknowledged and agreed that the email address provided or listed
herein shall be used for the purpose of complying with Section 553.791(15) of the Florida Statute.
The following items shall be completed prior to the release of any permit(s), as follows:

Written authorization from fee owner or fee owner's contractor for Private Provider which states the services to be performed by the Private Provider.

Notice to the Building Official (signed).

For both a Private Provider and any duly authorized representative: Copies of all documents necessary to satisfy the requirements set fourth by Section
553.791(1) of the Florida Statutes which includes, but is not limited to, Florida DBPR licenses under Chapter 471 as a Professional Engineer, Chapter 481 as and Architect,
or Chapter 468 as a Standard Building Code Administrator and Standard Inspector. -
Duly Authorized Representatives' Employment Affidavit (signed & notarized).

Private Provider Plan Compliance Affidavit (signed & notarized).

Affidavit for Building Code Inspection Services (signed & notarized).

Copies of the Private Provider's Certificates of Insurance.
All inspection reports shall be completely filled out and signed by the Private Provider or Duly Authorized Representative, In the event an inspection report is required
and not completed or signed, as provided herein, the City of Mulberry shall not accept same. -
